New Client Forms

This is the paperwork that you will need for your first appointment.  Please open, print and fill out to bring to your first appointment. Everything is a signature and date except for the Psychosocial history.  If you are not able to finish the history prior to your first appointment don't worry, do as much as you can and bring it with you so we can complete in session.
